When to use %d with EAC and Encoder

I've noticed that on some recommended command lines for Exact Audio Copy that %d is not included in the command lines of some encoders.  For example, on Case's EAC Guide, the Ogg Vorbis and MusePack command lines do not include %d while the LAME MP3 command line includes %d.  Do some encoders not need to have a destination file specified or does EAC handle the destination location for the file?  If EAC handles the desitination location for the file then why does LAME and some other encoders seem to need the %d?

oggenc foo.wav and you get foo.ogg
lame foo.wav and you get foo.wav.mp3, not foo.mp3

Maybe this is related?
