Updated Lame docs

Updated docs for LAME.

[Edit: Updated the detailed switches. Added a few missing and identified deprecated, and alpha/debug only ones]

[Edit2: Updated again removing all info about gpsycho, and creating new pages about cbr, vbr and abr. Also updated the detailed commanline list. Now that list is considered finished]
