What about classical music? I would assume a lot of (older?) classical CDs used indices above 01.

I remember the album by Focus - Moving Waves (aka Focus II).
The last track, Eruption had 16 parts. Originally it was side 2 of a vinyl LP that was divided in 5 parts (so sub parts grouped together).
In theory, there is no difference between theory and practice. In practice there is.

  • Erich Kunzel/Cincinnati Pops: Round-Up!, Happy Trails 
  • John Martyn: Foundations 
  • The Mystic Moods Orchestra: Extensions 
  • Octopus: Octopus [FAX label] 
  • John Oswald: Plunderphonic

  And a few unusual edge cases that might be useful for testing: 

  • In addition to lots of tracks with multiple subtrack indicies, disc 2 of John Oswald's 69 Plunderphonics 96 has a nonstandard TOC that starts with Track 27. That's a fun one to rip. 
  • Sigue Sigue Sputnik's Flaunt It has the inter-track commercials in Index 0, including on track 9, which has only four (silent) frames in Index 01 before the disc ends. CUETools can't handle that for some reason. 
  • Yello: One Second (Remaster) -- I'm not sure if this is really in the TOC or just a weird error that happened when I ripped it; my copy isn't handy so I can't double-check. But the EAC cuesheet shows Track 6 Index 2 and Track 7 Index 0 both at 26:24:40.
