Fade out, fade in, crossfade mix CD tracks

I've got a selection of tracks that I want to put on a Mix CD.  But some of the tracks end abruptly, so I'd like to fade those out.  Also, I'd like to crossfade/overlap some of the tracks.  How do I do this without a) creating "special" versions of songs that have fadeouts, and b) "gluing" multiple tracks together in one big track?  (I'd like to have the crossfaded tracks still register as tracks 5 and 6, if you follow me.)

Thanks!
