raw Mpeg2 from DVD

I have some dvds backed up in iso format on my HD and I've been trying to find a solution to rip the raw mpeg2 in its native bit rate, aspect, etc, and one or all audio streams into one file of my specification by chapters (so I can separate by episode).  Everything that I've tried has either split the audio into a separate file or tries to re-encode or re-format the video.  Help please  ...
It's due for a good DEGAUSSIN'

DGindex (comes with gordianknot package), open vobs then choose "file/save project and demux video" = one m2v and bunch of ac3 files (or whatever depending on audio extraction settings).
