The External Compression Placeholders can be used to pass information to the command-line options that are supported by the external encoder.
You didn't say what external program you're using. Are you sure it has a command-line option to embed the cover art?
For example: TAKC 2.3.0 only has a command-line option to add textual APEv2 tags. From the readme:

Missing features

Some important features are missing in this release. Please find below my current to-do list (in order of my current priority):
  •     Store cover art in APEv2 tags.
  •     Option to decode only a specific part of a file.
  •     Raw audio data files as input for the encoder.
[...]

wapet also looks like textual tags only.

http://www.exactaudiocopy.de/en/index.php/support/faq/ states that you can use %original% placeholder only in “Additional command line options” field, and not in the filename settings. So, what you want is not possible.
