Exception Track 2 INDEX 00 is 91:42:56 which is past the end of the source file

I'm trying to understand what is going on here.  I am ripping use EAC. It rips to FLAC tracks just fine, Accurip is succesful. But when I rip to image, it's giving several tracks that have these abnormal INDEX 00 numbers. This has only happened on this specific CD so far, but I have done about 6 discs using the EAC -> Cuetools method. I have not used cuetools alot, and EAC is in fact putting those numbers in the cue sheet. So I think Cuetools is doing what it's supposed to be doing, but I don't understand why the cue file is being made this way or how can I fix this. I tried changing EAC gap detection from innaccurate to accurate. I don't usually do the cuetools, but I have some scratched discs so I've been trying to use it to repair the rips.

To use this CUE in CUETools you'll need to remove all the INDEX 00 lines from Track 02 to the end. Do not remove INDEX 00 from Track 01.
I wouldn't be too concerned about fixing this in EAC unless you continue to notice problems. The best settings for this particular CD may not be best for the majority of CDs you RIP.[1]
